REGISTRATION INFORMATION
(Please read carefully)
1) Go to www.eayso.org. Log in, if you have not used eayso.org, you will need to register
2) Click on the tab about re-registering or registering your player for the first time, fill out info.
3) Click on the player name and verify that the information is correct.
4) Print 2 copies of the form, signing both pages (it's a two page form), please make sure to initial back page, this is the medical release and it needs to have a wet signature.

5) A check made out to "Millbrae AYSO" in the total amount in reference to:
1st child: $125
2nd child: $110
3rd child: $100
4th: $50
U5: $60
6) and the two signed forms to: Millbrae AYSO, PO Box 1461, Millbrae, CA 94030
7) Or you can drop the forms in the AYSO box at 249 La Cruz Ave., Millbrae, CA
8) IF YOU ARE REGISTERING A NEW PLAYER, you will need to include a copy of the player’s passport or birth certificate to verify date of birth.
9) We will notify you if there is room in the division your are requesting as soon as possible.

REGISTRATION FAQS
What do I have to bring to Registration? If you are a NEW player, you must bring a Birth Certificate or Passport as identification/proof of age. If you are a returning player, returning to U6 thru U10, you do not have to bring anything extra to Registration. If you are U12 thru U19, you need to bring a CURRENT PHOTO (1 x 1.5 inches) of your child. These teams travel to other cities in the AREA and are required to have Player ID cards. This regulation is strictly enforced and is required under AYSO National Guidelines.
Can I register my child on the Internet? You can pre-register your child on the Internet at www.eayso.com. You must print two (2) copies of the registration form, sign both copies, and bring them (in person) to one of the registration sessions to pay the registration fee. An Express Lane will be by provided to expedite pre-registered players.
How do I qualify for the Early Registration Fees? Register your child(ren) during the early registration period (by the end of the May 28th registration session) and receive a $25 per child discount off of the Regular Registration Fee.
How old does my child have to be to play? Players must be born on or before 8/1/09 to play in AYSO. See the table below for all division/age group assignments:

This age guide is for games and competitions that begin on or after Aug. 1, 2013.
What if my child has a birthday between 7/31/13 and the beginning of the season, or wants to 'play up' to be with his/her grade level? Players will initially be assigned to a division based on their age as of 7/31/13. Requests to 'play up' should be made on the registration form and will be considered based on available space in the higher division, as well as other circumstances. "Other" requests should be forwarded to the Regional Commissioner for review and consideration.
Can my child 'play down' in a division below their age group? No. The National Rules and Regulations do not allow a player to 'play down' a division in AYSO.
What can I do to ensure that my child has a positive experience in youth soccer? Become involved in the program. AYSO, in Millbrae and everywhere else, is a 100% VOLUNTEER organization. Without Volunteers, we don't have a program. We need POSITIVE coaches, referees, team managers / parents, folks to line the fields and maintain (paint) those lines, week in and week out, folks to run the snack bar, (after other folks shop to stock the snack bar) fundraising, photo day, etc. etc. etc. GET INVOLVED....BE A VOLUNTEER.
Do I have to volunteer? There will be a volunteer information table at the registration sessions. We will ask each family to volunteer in some way so think about what you would like to do to help this season. The more volunteers we have, the less each person has to do to help the program succeed.
